This form allows customers with existing permits to request a change, addition or cancellation for their bookings at facilities booked through Central Scheduling. Central scheduling will process your request within 5 business days and update your permit to reflect the change(s). A copy will be emailed to you once completed. Your request is not confirmed until you receive your updated permit or amendment.
