City of Regina | My Own Personal Information - Access to Information Request Form

For access to your own personal information, no application fee is required.

To access your employment records with the City of Regina an access to information request may not be required. City of Regina employees can access additional information and resources on our intranet, City Connect. If you are a past employee please submit an online service request or call Service Regina at 306-777-7000.

Personal information consists of information you personally provided to the City of Regina. Personal information does not include everywhere your name is mentioned in records nor does it include complaints against your property.

If you are requesting personal information and general information please split your access to information request into two requests: one request for Access to My Own Personal Information and one request for Access to General Information.

We are authorized by Section 6(1) of The Local Authority Freedom of Information and Protection of Privacy Act to ask for the information on this form. We will use this information to process your request.
